Uperio (Sacubitril and Valsartan) | 100mg 28 Film Coated Tablets is a dual-action heart failure medication combining a neprilysin inhibitor (sacubitril) and an angiotensin II receptor blocker (valsartan) in each 100 mg tablet (49 mg sacubitril + 51 mg valsartan), specifically designed for adults with symptomatic heart failure and reduced ejection fraction (NYHA II–IV).

Usage & Dosage:

Initiation: 100 mg twice daily for patients transitioning from ACE inhibitors/ARBs, or not currently on ACE/ARB.

Titration: Increase every 2–4 weeks to a target dose of 200 mg twice daily, as tolerated.

Special Populations:

Renal impairment (eGFR 30–60) or mild hepatic dysfunction: start at 50 mg twice daily.

Contraindicated: Severe renal/hepatic impairment, pregnancy, ACE inhibitor overlap (36-hour washout).

Contraindications & Warnings:
- Do not use with concurrent ACE inhibitors (due to angioedema risk); maintain a 36‑hour washout period.
- Avoid in pregnancy, hereditary/idiopathic angioedema, severe hepatic impairment, concomitant aliskiren in diabetics/CKD.
- Monitor for hypotension, hyperkalemia, renal function decline, and rare angioedema—especially in Black patients.
Common Side Effects:
- Low blood pressure, hyperkalemia, cough, dizziness, and renal impairment (>1%).
- Rare but serious: angioedema, nephrotoxicity
